Chuck M. Crush, LNHA,  Administrator

Chuck has been in Healthcare Leadership for over 25 years and has led Skilled Nursing Facilities, Hospices, and Assisted Living Communities in Virginia, New Hampshire, and Massachusetts. Chuck holds a bachelor’s degree from Roanoke College and is a graduate of various leadership programs including the Geisel School of Medicine Leadership program at Dartmouth College as well as the Disney Institute Leadership Excellence program. He is also a Certified Community Coach, with training from the Fowler International Coaching Institute. He holds the prestigious certification of a (CDAL) Certified Director of Assisted Living and is also a Licensed Nursing Home Administrator in Massachusetts and New Hampshire. (LNHA) Chuck lives in Concord with his spouse Amy. Chuck has three children ranging in age from 31 to 17. He loves reading, traveling and enjoying good wine and food. He is excited to be part of the Salemhaven family. He very much enjoys connecting with the residents, staff and greater community.

 

 

Carolyn Farhadian, RN,  Director of Nursing Services

Born in Lawrence MA, Carolyn moved to Salem, NH at age six and was educated in the Salem School system. She now resides in Hampstead, NH with her husband Tom. They have two sons and four grandchildren. Carolyn started her career in business as an administrative assistant at Pinkerton Academy. Striving for a new career, she chose nursing and received her RN from Rivier College. Carolyn has been with Salemhaven for 9 years and has 13 years of experience in long-term care and hospice.


 

Juanita Dilendick, RN, Assistant Director of Nursing Services


William "Bill" Pitocchelli, Admissions Director  

Bill has been in the nursing home industry since 1989. He has been the Director of Admissions in the Merrimack Valley for many years. He also worked as Corporate Regional Director for Census Development/Insurance Contracts locally. Bill has a bachelor’s degree in criminal justice from St Anselm’s College and an Associate’s Degree in Accounting from Northern Essex Community College.



Allison Battis, MSW,  Director of Social Services

Allison has been the Director of Social Services since February 2015.  Allison’s role at Salemhaven is to address any emotional or social factors relating to the residents and their current situation. She is available to identify and handle any issues that may arise during the length of stay at Salemhaven for both the residents, as well as their family members. Allison’s focus is on the psychosocial well-being of the residents to ensure their time with us at Salemhaven is enjoyable.  Allison graduated from Merrimack College in North Andover with her bachelor’s degree in health science/Sociology.  Allison went on to obtain her LSW license in 2001 and has been working with the geriatric population since 1998 in both home care and long-term settings.  Allison lives in Atkinson NH with her husband and has one daughter. 



Jim Burchell, Director of Dietary Services

Jim Burchell has been in the food service industry for many years.  He graduated from Johnson & Wales University receiving an associate’s degree in culinary arts in 1983 and a bachelor’s degree in food service management in 1985. After working as a Sous Chef for the Sheraton Corporation in San Diego California, he relocated back to New England to work in a number of fine dining restaurants. In 1996 Jim began working as Director of Food Services at skilled nursing facilities in the area. Born in Haverhill MA, he currently lives with his wife Sandra in Kingston NH.
