Social Services 

Salemhaven Social Services supports the psychological and social well-being of each resident within the facility. These services include:

• Psychosocial support for the residents, as well as their family members. 
• Educating the resident, family and friends about the residents’ rights. 
• Participating with a multi-disciplinary team to develop a comprehensive plan of care for each resident. 
• Supporting the resident’s family in coping with the physical illness and/or disabilities of the resident and the grieving process. 
• Working with residents and family members and other disciplines to have a safe plan of discharge to home or another level of care. 
• Providing community resource information regarding discharge planning.
